Long-term care management....This service provides oversight and management of all aspects of client needs. Our service embraces medical needs but is not limited to just that. We are able to assist with other needs the client may require such as, transportation arrangements, socialization, household and car repairs. The care manager has knowledge of local resources in the community, which will assure quality of service and cost savings.

Complete assessment with report.... Primary prevention is the first step and we advocate this for every client. This service provides the client with a report that identifies and evaluates their current health, functional, psychosocial and safety needs. It allows the care manger to identify potential problems to prevent incidence of crisis and allows the client to maintain or improve their current level of functioning. This service is the most important; it saves lives and saves money.

Placement assistance....This service provides the client with a brief assessment of level of care needs which includes a review of their current situation. An in depth written report on 2-3 appropriate facilities is prepared and given to the client. An experienced RN, to ensure that information given to the client is current and accurate, visits each potential facility. (Further assistance with transfer arrangements is also available.)

Review of requested facilities with a written report...This service provides the client with updated information on facilities. Although, you can get this information from other sources, we guarantee that an experienced RN will visit the requested facilities at the time of inquiry. Facilities are continually changing so this ensures accurate reporting.

Medical record review....This service provides an independent review of all available medical records by an experienced RN. Health care today can be very complex and fragmented with a client receiving care from various providers. This service can help assure accuracy and enhance coordination of care.

Welcome Home services....This service provides assistance with seasonal clients to assure that their home is ready for their return. We also make medical appointments and any other arrangements that are needed for their return.

Medicaid spend down assistance....This service assists clients with all required documentation for Medicaid filing.

Shopping coordination....This service provides coordination of shopping needs. Many clients are on special diets. We coordinate the purchase of all appropriate foods for the client to assure proper nutrition and hydration. Malnutrition and dehydration are one of the major causes of hospitalization in Florida.
